Meals on Wheels & Case Management

Last year Meals on Wheels Orange County prepared and delivered 663,345 meals to homebound and frail older adults. Thanks to Meals on Wheels Orange County case managers, you also receive referrals to additional supportive services such as health and personal care, chores assistance, and home safety. Meals on Wheels Orange County administers one of the largest Meals on Wheels Programs in the nation, giving Orange County older adults avenues for staying healthy and living independently.
Senior Center Lunches

Good nutrition is essential to keeping older adults healthy and thriving. Meals on Wheels Orange County delivers nutritious meals to 23 senior centers throughout central and northern Orange County, providing more than 1,500 seniors at least one-third of their daily nutritional needs each weekday. Our congregate lunch program offers both nutrition and social connections needed for physical and mental well-being. Come to your senior center and lunch with us.
Adult Day Care Services

We keep families together by providing a warm social environment, safe supervision and therapies 5 days a week for adults with physical disabilities or mental impairments. Meals on Wheels Orange County’s three facilities provide a caring environment and a wide variety of social opportunities, nutrition, and monitoring during the day. Our focus on person-centered care means we tend to both participants and caregivers. Support for family caregivers means everyone is healthier and happier.
Friendly Visitors

More than 600 Meals on Wheels Orange County volunteers create a network of care and support for older adults in Orange County. Our Friendly Visitors provide companionship, friendship, warmth, acts of service and a sense of social connection and well being to someone who is isolated or lonely.
Care Coordination

Supporting those recently discharged from the hospital or nursing facility, Care Coordination is a suite of services designed to strengthen patient health and healing and reduce risks associated with re-admittance. In-home assessments, coaching, early response to health risks and symptoms, medication reconciliation, telephone support and health communication with health care providers help ensure strong and healthy recovery.
